The way a black hole forms is via really massive stars. When these neutron stars' masses are greater than 2-3 the mass of our sun, their respective neutron degeneracy pressures (think of it as each star's internal engine) cannot support them any longer. The star then collapses. If the matter is compressed so much, due to its own gravity, that the escape velocity (the speed needed to escape from the gravitational pull of the star) is equal to or greater than the speed of light, then nothing can escape, not even light.
